Keep in mind that a "cowboy snaffle" with long shanks is not 
a snaffle at all, but actually a curb bit with the added 
harshness of having all that leverage to "nutcracker" the 
lower jaw.  It is about one of the most painful bits I can 
imagine.  I can imagine it was just excruciating for her, 
given the pathology present as well.

It depends on the amount of force applied. I've seen riders bloody a horse's
mouth with a regular snaffle. We had a horse that was trained to a Tom Thumb
before we got her, and it worked very well. You just didn't pull very hard,
ever. Didn't need to. It would not be my first choice to train a new horse,
though. I definitely prefer a hackamore for endurance - I think it helps the
horse eat and drink better.
