First, I would have a complete heart work-up done...

But, I used to have this and it was related to gas pain/pressure and was related to what I ate before and during a ride. As much as I love peanut butter, it does that to me. And never mix fruit and bread products on the trail; that can give you gas too.
I used to carry Maalox tablets with me in tin foil- if I got gas I'd just pop two or three and be fine in a few minutes.
