She does have a large hay belly, could it be that she is having some
head/neck problems that keep her from chewing her food properly? She is
on good forage.

Most likely problem is a dental issue.  Hooks and waves that keep the
dental arcades from freely moving across each other can cause discomfort
and a lack of ability to properly position the head, and hence difficulty
in moving the entire spine.

And poorly-chewed food could contribute to the "hay" belly, too.
