No rider may receive an RO of any type UNTIL his/her horse has been cleared by the vets to continue.

That some have been given RO's before an exam or despite an exam is the result of ignorance on the part of the vet.

Here is the pertinent quotation from "Veterinary Guidelines for Judging AERC Endurance Competitions", aka the Vet Handbook:

Please note: RIDER OPTION, RIDER OPTION-LAME and RIDER OPTION-METABOLIC are only to be used in cases where the horse has cleared/passed veterinary inspection and is fit to continue, but rider elects to withdraw.
