I don't have experience with equine mane or skin problems.  I do have some
experience with being blown off!

I'd try that Ivermect treatment and not even talk to that vet.  Give the
Ivermetin treatment a chance to work.  If it does, then find out if you need
to treat the property.  Or if it doesn't, look for an openminded vet who
doesn't worry about his ego being challenged!

Just because they have the DVM (same goes for MD) doesn't mean they have
seen it all.  Experience is quite the teacher.  The credentials are not what
make the MD/DVM nor where it is from, not usually.  My own personal
physician taught me this.    

You might try contacting other Equine Universities and asking for
consultation, ask if anyone has seen problems like this and what their
findings/treatments were.  Or even try finding a vet with a specialty in
equine skin and hair problems, a dermatologist.
