I had a vet tell me that he grades "A" if he sees or hears what he thinks is normal for a horse having gone 15, 35, 50 miles (or whatever the breakdown of the loops is). So he looks at the horse for a pre-check as a horse at rest and grades against that. At the first check, he grades based on what he expects from a horse that has gone 15 miles and so on.

One thing to keep in mind is that vets vary a bit in what they consider an "A" vs. a "B" on gut sounds.  One vet's A might be an A- or a B+, or even a B to another vet.
