EIA is most prevalent in the Mississippi river valley, TX and Florida.  When testing first became available, some Florida TB farms had 25% or more positive.  The reason turned out to be (in part)  that the horses were sharing needles just like human drug addicts.  (EIA is the same type of virus as the one that causes HIV, though EIA CAN NOT infect humans).
 
30 years ago some people wanted to test all US horses and require slaughter of the positives.  For various reasons this was not done.  The result is that instead of EIA being extinct like Glanders, we still have to test millions of horses each year to keep it in check.
 
By the way, even though there are some horses that do not get sick, many do get very sick.  Some die, some recover and then get sick periodically all their lives.
