What you need is a small hole cut in a pad to allow the leathers to clear
the skin.  You have to much pressure there, you need to relieve it.

Putting on a heavier pad would be like putting on an extra sock to prevent
your shoe pressing on your bunion.   Cutting a hole in a pad would be like
cutting a hole in the shoe.
