Lots if not most foot surgeries can be done with a local and on an
outpatient basis.  In the early 70's I had surgery for a mortons
neuroma-it was highly succesful and no problems there since.  My feet
are my weak point too and all those years of backpacking and climbing
took their tole.  I have wide feet and I love the Ariat Terrains.  I was
actually able to order my regular size and not have to get the mens.
That and easy ride stirrups really make a difference in how my feet feel
at the end of a ride.

I don't think you have to join a gym or buy frilly outfits to get some
fitness.  A decent set of shoes and a 15 minute dismount here and there
will help.
