It is possible that back pain could contribute to your horses
tripping, but it would not be the furst thing I would rule out. 
You mentioned that you & your farrier are working on shortening
his toe and getting some heels on him: could be too much angle
change has been done too fast: you might have your farrier back
out for an assessment.  I'd also want to have a vet out to see
if it looks like an neurological issue (such as EPM).  Given the
severity of the stumbling, I'd get a vet out ASAP, and give him
some time of (at least 2 weeks) to rest, and see if there is any
improvement after that.
