Along the PT lines, Pete Egoscue's methods (www.egoscue.com) have helped me
more than anything.  Not all PTs are very exercise oriented (although some
are and can't be beat!).  I had one just give me orthotics (and NO exercises
to my disappointment) when I was having hip/foot (related) problems.
Started doing some Egoscue exercises and everything is back where it should
be.  For general fitness (and chronic injury) I also find that Yoga helps me
stay healthy more than weight lifting (although I do both - I really need
the stretching I get in Yoga).

I have to second that..  In the late '70's I developed back problems.
After
9 months and a number of chiro's who could give symptomatic relief but no
cure, I found a good sports medicine orthopaedist.  One exam, a
prescription
to the physical therepist, and a year of exercises and my back has been
good
since.

The keys are:

1. Sports medicine.  Ordinary doctors don't believe in PT because so many
of
their patients lie and say they have been doing their exercises when they
haven't.  Sports med. Drs. know, because their motivated patients follow
their advice, that PT works.

2. Time.  PT does not work instantly.  You may find 3-6 months of work for
large benefit, a year or more for maximum effect.  It is like LSD in
horses.
