There is a very simple way to avoid losing a horse while switching from
halter to bridle: Don't take the halter off!

Use either a thin rope halter....or a biothane halter with clip on bit....or
halter with add-on headstall that consists of poll strap
with browband that clips around the halter.

I would never, ever consider riding on the trail without a halter on
underneath.  Then you have it to tie to on the trail.

Why on earth take the halter off?
