Take a look at the winning Individual Gold dressage ride in Hong Kong (Isabella?). During the piaffe, the horse balked, blew up, backed up, and very nearly ended up outside the arena! I never did figure out why that ride got gold.... Debbie McDonald's ride on Brentina was apparently a disaster, with the horse spooking and totally losing her cool. So - it does happen, though not to the extent described for the WEC. :)
