Many years ago, our daughter was riding her horse in the forest and he tripped over a redwood log and received a puncture wound. Anyone who knows redwood knows that it causes festering, as with a splinter in the finger. This horse received a large splinter of redwood in the tendon area, and although we treated it, it kept on festering, healing, opening, festering for about a year. Finally took the horse to a vet and he sliced open the skin in the area of the hole and drew out the splinter. After further flushing, the wound healed perfectly.

Wondering if anyone has experience with a scar on their horse that
seems to get infected every few months..

I was coming down the mountain on Kaboot through a trail so washed out it looked more like a stream bed. Got off at the bottom and his right rear fetlock was laid open. Had no idea when he did it. I healed, but one corner stayed open and kept draining. Finally took him to the vet and he kind of opened it up and flushed it good (really good) inside sort of below the wound. His theory was that he had cut it on the upswing and the debris was below the opening and wasn't able to drain up to get out of it. Since you said this opening is at the top of the old wound, maybe the same happened to yours.
