With as many times as you have gotten back on and with the injuries you have had, if you have "fear issues", I've got paralyzing terrorizing horrific horsophobia persecutory delusions with acute psychosis. But I ride an appy so it is to be expected.

I used to fall off him regularly but we get along much better now. Luckily, I never had worse than the wind knocked out of me, yet. My wife got a spinal fracture this spring but is mending nicely. The big question is how much is riding THAT horse worth to you. A lot of folks would have sold or alpo'd him by now. A lot of folks would say you are just getting started with him. I SHOULD have canned my appy 6 months after I bought him and most reasonable people would have but two-and-a-half years later, I can say I am immensely glad I did not and wouldn't sell him now for anything. He is my friend. Sometimes, friendship hurts but that doesn't make it not worth it, at least to me. Good luck. It is a hard decision.
