We have Heartworms pretty badly in Mariposa and so using a med is almost a necessity. My vet has a client whose Jack Russell contracted Heartworms. His owner said she religiously gave her dog its monthly pill and couldn't understand how it was possible that he had Heartworms. Totally perplexed, she went home and literally tore her house apart... and there, under the living room sofa, was a nice, neat pile of about 30 pills!
