Yes, it means you will get credit for the miles on your horse's mileage record, but not be in the running for awards at the ride or year end awards based on points.
 
MOTDRA also allows bares and other over-the-hairline boots for mileage only. I think NATRC has a division in which you can ride just for the miles.
