I have a two year old Polar heart monitor with the thin sensors.  When I get real high readings, it's because one of the electrodes is not reading.
 
Being new to the sport, this initially freaked me out.  I would look at my watch, think "Holy moley, he's at 220."  Then, a high-mileage friend would remind me to check and I'd see the thin pad swinging in the breeze.  Of course when I learned to secure it effectively, this quit happening.
