Has he had his sheath cleaned lately? He may have a "bean" . That would be a ball of smegma lodged in the end of his you know what. My gelding gets them quite often. When I first got him I don't think anyone had ever cleaned him. He had a bean so big that it was hard for him to pee. It would just come out in little squirts.
Worth a look see.
