My veteran Arab Drubin had needle phobia for many years, like until
his teen years! Acupuncture was a particular challenge.
Our two most recent vets, both women (hmmm), Dr. Jeannie
Waldron in Va. and Dr. Ann Stuart in NC, have managed to get Drubin over
this fear (or at least close).  The technique seems to be to gently pinch
the skin and then slide the needle in.  So would recommend you try gently
pinching your horse during grooming and the pat and reassure no big deal.
Do this on a regular but not predictable basis.
Then the next time she needs a shot try gently pinching (or have vet
do same) and then sliding needle in.  Hope this helps.
