Now what PR person is going to catch an arrthythmia? If they got directly to the vet he would catch it what maybe a minute later. They don't go back to the PR person after they see the vet.

I guess I am confused here.

by the vet--I think that is true everywhere. What I mean is that it can be caught earlier--by the PR people--if they use a stethoscope. For example, if we were to adopt Heidi's suggestion of exit exams, it wouldn't be caught until the horse had been in the check for quite a while.
