This is nothing new....I tried putting foam between the pad and the sole at the
heel and it helped.  You are best off putting the silicone on the foot BEFORE
nailing the shoe and pad on.

I gave up, eventually, and use easyboots over shoes.  Cheaper in the long run
and less hassle.
