My old vet (who did a lot of race horses) said that the
biggest mistake people make is tieing the horse's head
up.  This is what causes "shipping fever," the pneumonia
that takes the lives of so many horses shipped
long distance without respite.  The horse has to be able
to get its head down to drain mucous and dust, from both
the road but most particularly from hay in a bag next
to its head.  Otherwise,  everything drains into the lungs.

And a big loose square box is best, counter to my every
intuition that this would make it hard for the horse to keep
its balance.
