Mary:  it is a complex issue with many factors.  Anything that affects the smooth movement of the joints will cause TMJ pain.  So teeth can be a big factor, as well as rider hands, bits, genetics of bone development, feeding the horse up rather than off the ground, etc.  You  were lucky that the TMJs settled down after the dentistry and bute and they didn't need to be adjusted.  Some are so locked in to place, they need some kind of body work, whether chiro, massage, cranial-sacral or acup, after dentistry to help them return to a point of balance.
