He probably did have to. I had to sign a similar document, and get 
authorization from the state veterinary health folks before Ft. Dodge could 
ship the vaccine when it was first released. Until very recently, the 
vaccine had only limited approval, and  it's use was restricted. Some 
states allowed DVMs to dispense it to clients, others mandated that DVMs 
only administer it.
It is now fully licensed, so that shouldn't be such an issue anymore.
