Stepping on nails...

One of our neighbors just had a rather awful incident where her horse
stepped on two upturned nails, piercing the hoof just over an inch away from
the frog on either side of the frog, towards the toe. The horse is falling
down lame {of course} and there is blood showing in the two holes, though
it's not still draining. 

I went over and looked at it with her, she doesn't want to call the vet, as
she feels it will heal fine since they pierced the hoof outside the frog.
She says it's almost like being 'quicked' by a farrier. I am not so sure
about this. obviously the nails penetrated deep enough to cause severe pain
and visible bleeding down through the hoof. At the very least I would be
concerned about abscesses {she says she'll dose him with penicillin to
prevent this}. Tetanus would also be a concern - however she says he is
vaccinated.

Is there something she needs to be further concerned about? The horse is
young {2} and not under saddle.
