I think if this was my horse and I had all the problems that you have had i
would put him on a long picket line where he could not rub on any thing.  Even
if this meant having to water him by hand several times a day this would seem
easier than to have to repeatedly have his his face stitched up.  I would also
put a cradle around his neck and put a muzzle on him.  If this seems harsh just
think of what he is doing to himself.  Good luck!
