I did use wedge pads on one gelding that had a LTLH problem.  I made the
farrier use a wedge, 3 degree at first, on the foot with no heel.  He
set back the shoe to where we wanted the foot to grow, squared and
beveled back the toe, and used the wedge pad to fill in where the heel
should have been.  It worked wonderfully...seems that the pad stimulated
circulation where the heel needed to be, and actually caused a wonderful
new heel to appear where there had been none before.  We re-shod him
each time with less wedge, a 2 then a 1 then no wedge at all.  In a
fairly short time he had a good heel growing, and no more runaway toe.
