Linda;

I would start out by finding a good sports medicine MD, DO or DC to take a
look at your ankle. The fact that you re-injure it each spring is a big
concern. Two things could be happening when you think you are in condition:
1. You are stronger and able to support the ankle. Question: "why do you
atrophy so much in the off season that you lose the support?" 2. Your body
has compensated for the ankle in some other structural way, pelvic shift,
weight transfer to other leg ... and your horse has to adapt to this shift as
well.
Enough soap box.
A simple exercise, that we can do: Lay on your back, with your legs extended.
SLOWLY raise your toes towards your head, then raise your foot flexing at the
ankle, let you knee bend if it wants to. Then relax your foot, after your foot
relax your toes. Repeat 5 times. Do this exercise whenever you need to but at
least AM/PM.
You can modify it to use while sitting, or riding.
This exercise will recondition the intrinsic muscles and nervous control. You
are probably using the extrinsics since your injury.
Please go find someone that can help you with re-aligning your body and
getting that ankle working properly.
