 My six y.o. TWH gelding had a serious 
problem about a year and a half ago.  He stretched out continually 
before and after he peed.  Turned out he had a serious bladder 
infection.  This was NOT diagnosed in the urinalysis, or the ultrasound, or 
the physical exam by the vets.  It took a scoping at the clinic.  And 
the vet said he probably picked up the infection because his auto waterer was 
filling TOO slowly so he wasn't drinking enough to pee enough to flush out 
the bacteria that normally reside inside the sheath, penis, bladder, etc. I 
suggest a thorough exam by a competent vet. (And monitor horse's water 
consumption.) Hope this helps.
