swollen belly from sheath on 19 yr old gelding

Help!  I just noticed that the belly of my daughter's
gelding has a hard swollen patch extending from one
side of his sheath forward to his belly button.  We
cleaned his sheath and business (only have to stroke
the sheath to get him to drop it - x stallion), but it
hasn't gone away.  What could it be?  It is a hand
wide and about 15" long...
