Hold criteria

Some latitude is observed from region to region of the US, but for the most part
they are basically the same.  The 10 minute times I quoted should not have been
put in since this is an internal procedure that is only used in special
circumstances. Normally in every situation the total down time is hold plus
pulse with hold time starting when pulse criteria is met.
