heart attacks in horses

The next door neighbors of my parents are die hard trail riders on TWH, they 
go every weekend. A few years back the were climbing up a hill and the wife's 
horse suddenly had a heart attack fell over backwards, landing on her and 
both horse rider were dead instantly. There had been no previous warning.
