Sorry for the previous blank message--my computer gets this "thing" sometimes 
and does that, and I have to reboot...

But--YES, there is need for further concern about this puncture!  ANY sole 
puncture which draws blood has gone deep enough to invade sensitive tissues 
with may or may not include structures such as the plantar cushion, the 
navicular bursa, the navicular bone, the coffin bone, etc.  Especially since 
you describe this youngster as being extremely lame, I would be VERY 
concerned that one of these major structures has been involved.  At very 
least, the risk of infection and abcessation is great.
