I have a pair of leather ice boots with a single large pocket in which I
place crushed ice (that's why there's a hammer in the vet check kit--it's
not just to hit Kevin over the head!). When Mr T comes in, he floats
through the P&R's, does trot-out & I immediately place the boots on the
front legs (they velcro close with stretchy fabric). I leave them on for
no more than 5 minutes, then switch to the back legs and let the front
warm up. If the amount of time in the hold permits, or if Kevin
wants to just hang out for awhile, I will alternate the 5 min on-5 min
off routine for a total time of 20 minutes. If ice is not available I
will continue to flush the legs with cool/cold water throughout the hold
time. Kevin usually goes out of the hold at a nice 7-9 mph trot (slow
for Mr T) and doesn't start to boogie until he is well warmed up.

This seems to work for us!
