Ed, when pull codes were first envisioned, a questionaire covering these
things was also developed.  For some reason, the pull codes were adopted,
but not the rest of the system.  It was always my hope that the rest would
gradually be phased in, but if people are so resistant to writing down
something as simple as the pull codes, it's hard to envision getting
cooperation to gather the rest of the data.
