Has your DVM done any bloodwork on the horse? I'd strongly suggest at least 
a CBC/chem profile, and then perhaps more "exotic" stuff based on what, if 
any, abnormalities are found there.
How about a Coggins test?
Have the teeth been looked at/floated?
(BTW, the protein in your current diet ought to be adequate for a normal horse)
