Here is a suggestion for ride managers towards meeting that goal of having
riders carry medical info with them...I have often seen them give vet wrap
or sponges or some small thing like that for completion awards.  How about
giving either the arm thing, or a pouch type thing that pins on or the
necklace doohicky as a completion award to encourage riders to carry that
info with them at all times.  I wear my medic alert bracelet or necklace
that indicates I am a diabetic but it does not have the info such as blood
type or medications, etc.  I think what the 3 day eventers do is quite wise
and we can always have a copy of a living will or written instructions for
"no extraordinary measures" included with such info.
